Instructions - Joint Application Section A, Part 7

Part 7 requests information on previous permits and project meetings, and address information for public notice requirements.

A.  If there have been any pre-application meetings, including on-site meetings, with regulatory staff, please list the date(s), location(s), and names of key staff and project representatives.

Pre-application meetings are strongly encouraged.  They help familiarize Department staff with the project as well as help the applicant and/or agent know what to expect and what may be required.  They are especially important for large projects or projects including 10 or more boat slips because of complicated testing, siting and possible State-owned Lands requirements.  Meetings for smaller projects are also encouraged, especially if you have questions about the process.  Pre-application meetings help to expedite the permitting process.  If there has not been a pre-application meeting for your project, please contact your nearest district office or branch office to schedule a meeting.

If there has been one or more meetings, enter the names of key agency staff and project representatives (for the applicant) present at the meeting, the agency, meeting location and a summary of the subjects discussed.  Use additional paper if necessary.

B.  Please identify by number any MSSW/Wetland Resource/ERP/ACOE Permits pending, issued or denied for projects at the location, and any related enforcement actions.

List all pending, issued or denied permits for this project location, and all past enforcement actions, even if they are not associated with the currently proposed project.  If there are none to the best of your knowledge, enter N/A.

Attach a copy of the listed documents, if available.

C.  Note: The following information is required for projects proposed to occur in, on or over wetlands that need a federal dredge and fill permit or an authorization to use State-owned Submerged Lands.  Please provide the names, addresses and zip codes of property owners whose property directly adjoins the project (excluding application) and/or (for proprietary authorizations) is located within a 500 ft. radius of the applicant's land.  Please attach a plan view showing the owner's names and adjoining property lines.  Attach additional sheets if necessary.

If your project does not require proprietary authorization (authorization to use State-owned Submerged Lands), then provide the names of the property owners whose property directly adjoins the project. Include a map showing the owner names and locations of adjoining properties. (Example)

If you project requires proprietary authorization, such as to build a dock, pier, marina or seawall, provide a list of property owners within a 500 foot radius of the project area.  Include a map showing the owner names and locations of adjoining properties. (Example)  Note:  This is not required for projects that qualify for consent by rule.

Read the document "Proprietary and Regulatory Authority" for more information about using State-owned Submerged Lands.

Maps, names and addresses are available from your county property appraiser's web site, or you can get the information in person from your local property appraiser's office.
