A. Signature block for the agent or applicant.
This signature block is for the person who is authorized to negotiate for and receive a copy of the permit.
If the applicant is applying directly, negotiating and receiving the permit, the applicant must fill out and sign this space completely.
If an agent (such as a consultant, engineer or other person associated with the project) is applying, negotiating and receiving the permit for the applicant, then the agent must fill out and sign this space completely.
B. Signature block for the applicant, if an agent signs A.
If an agent signs A, above, then the applicant must fill out and sign B completely.
This space authorizes the agent to negotiate and receive a copy of the permit on behalf of the applicant.
C. Signature block for the person authorizing access to the property.
This signature block is required to grant the Department, Water Management District and the U.S. Army Corps of Engineers access to the project site in order to inspect, review and sample the lands involved in the project for application review purposes.
Block C must be signed by the property owner or someone who can demonstrate legal authority to allow access to the property.
