All required documents must be submitted on 8.5" x 11" (letter size) paper.
A. Provide results of any wildlife surveys that have been conducted on the site, and provide any comments pertaining to the project from the Florida Game and Fresh Water Fish Commission and the US Fish and Wildlife Service.
These may not exist for your project, but if there have been any surveys done for previous applications or local government approval, or if there have been any comments from other agencies regarding the property, even if it was for a different project, provide a copy of any existing documents.
B. Provide a description of how water quantity, quality, hydroperiod, and habitat will be maintained in on-site wetlands and other surface waters that will be preserved or will remain undisturbed.
If on-site wetlands and surface waters are to be preserved or remain undisturbed, then efforts should be made to ensure that they will not be affected by the remainder of the project. Stormwater ponds, ditches or other drainage features should be constructed in a way to avoid drainage of the wetland or surface water, and also to avoid impoundment or any other adverse impact to natural drainage patterns. Habitat and water quality should not be degraded by destruction of native plant life or by pollutants being introduced through untreated drainage into the wetland or other surface water.
Provide a narrative describing how this will be accomplished.
C. Provide a narrative description of any proposed mitigation plans, including purpose, maintenance, monitoring, and construction sequence and techniques, and estimated costs.

D. Describe how boundaries of wetlands or other surface waters were determined. If there has ever been a jurisdictional declaratory statement, a formal wetland determination, a formal determination, a validated informal determination, or a revalidated jurisdictional determination, provide the identifying number.
The limits (boundaries) of wetlands and other surface waters must be delineated according to Chapter 62-340, F.A.C. and clearly shown on the drawings. Describe how the limits were determined.
Examples: wetland boundaries were field flagged and surveyed, located with GPS points, determined by agency staff, determined by a consulting firm.
The Department has several methods of officially validating a wetland line (for a fee) that are binding on the Department for a certain amount of time. These services are not available in all areas due to limited staff time and resources. If one of the above listed documents exist for your property, provide the identifying number and a copy, if available.
E. Impact Summary Tables:
For all projects, complete Tables 1, 2 and 3 as applicable.
